Getting Started. What You Will Likely Need…
- Your file (you can get it at clerk’s office 700 W. Jefferson St., Judicial Center, 1st floor Family Court Clerk or 600 W. Jefferson for District Criminal information such as bail, arraignment date and time, etc., next court date)
- Your ID- Drivers License or Personal Identification Card
- Case Style and Case Number if case pending
- Maiden name/DOB/Social Security numbers, phone numbers/street addresses for you, spouse/other parent and children
- Highest level education for you and your spouse
- Last two year’s tax return (state and federal)
- W2 statements, 1099 statements or proof of other earnings
- Investment account quarterly statements
- Deed
- Mortgage
- Mortgage payoff (including home equity line)
- PVA value of real estate or recent appraisal
- Most recent statements for bank accounts and debt owed and to whom
- Blue book value of vehicles
- Proof of tax liability (if any)
- Day care expense (work related only)
- Cost of health insurance for children only
- Name of business and EIN
- How many members/partners and names and addresses and phone numbers and social security numbers
- Articles of incorporation/partnership agreements
- Business tax return (corporate and partnership)
